How FOXO4 is described to act.
FOXO4-DRI is a retro-inverso, all-D-amino-acid peptide modelled on the FOXO4 region that contacts p53, fused to a cationic cell-permeability sequence. Solution NMR structural models show that the disordered peptide binds the disordered p53 transactivation domain (p53TAD2) to form a transiently folded complex, with both the FOXO4-derived region and the cell-permeability peptide contributing to the interaction, and with p53 phosphorylation increasing affinity for both FOXO4 and FOXO4-DRI. Senescent cells are reported to depend on a FOXO4-p53 interaction that retains p53 in the nucleus for their continued viability; the peptide perturbs that interaction, producing p53 nuclear exclusion and cell-intrinsic apoptosis. Non-senescent cells are reported to be comparatively unaffected, which is the stated basis for classifying the peptide as a senolytic.

The originating study identified FOXO4 as a pivot in senescent cell viability and reported that, under conditions where it was well tolerated in vivo, the peptide neutralized doxorubicin-induced chemotoxicity and altered fitness, fur density and renal function in fast-aging XpdTTD/TTD and naturally aged mice. In naturally aged mice, FOXO4-DRI selectively induced p53 nuclear exclusion and apoptosis in senescent Leydig cells and altered age-related testosterone secretion, with FOXO4 nuclear translocation observed in aged human Leydig cells. In human chondrocytes expanded in vitro, the peptide removed more than half of the cells at population doubling level 9 while not significantly changing cell number at the minimally expanded PDL3 control. In keloid organ cultures and fibroblasts, treatment promoted apoptosis and decreased G0/G1 phase cells alongside nuclear exclusion of p53 phosphorylated at serine 15. Countervailing data reported in Circulation found that clearing senescent cells, including with FOXO4-DRI, promoted rather than limited pulmonary hypertension development and progression in animal models, indicating that senolytic outcomes are tissue- and context-dependent. No human clinical trial of FOXO4-DRI appears in this citation set.
