How Sermorelin is described to act.
Sermorelin is GHRH(1-29)-NH2, described as the shortest synthetic fragment of human growth hormone-releasing hormone retaining full biological activity of the parent hormone. It binds the GHRH receptor on anterior pituitary somatotrophs and stimulates synthesis and release of endogenous growth hormone, so its output depends on an intact pituitary somatotroph population. In normal men, intravenous bolus dosing produced integrated plasma GH responses of molar potency similar to GHRH(1-40) and GHRH(1-44), while a D-Ala2 analog was approximately twice as potent. Reported endocrine selectivity was high in that study: no changes were observed in prolactin, TSH, LH, FSH, ACTH, insulin, glucagon, glucose, cortisol, or free thyroid hormones.

Because release is pituitary-dependent, the literature characterizes sermorelin substantially as a diagnostic provocative agent. Review data in children report that fewer false-positive GH responses occur after intravenous sermorelin than after other provocative tests, while noting that a normal response cannot exclude growth hormone deficiency of hypothalamic origin. A randomized 6-month pediatric study assigned 43 prepubertal children to low-dose GHRH(1-29)-NH2, high-dose GHRH(1-29)-NH2, or growth hormone, with anthropometry, bone age, GHRH provocative testing, and IGF-I as endpoints. In older adults, a 6-week study of nightly subcutaneous GHRH(1-29) in 11 healthy men aged 64-76 measured overnight GH secretion at 20-minute intervals, IGF-I, IGFBP-3, GHBP, muscle strength and histology, forearm muscle bioenergetics by 31P-NMR spectroscopy, and body composition by DEXA. Twice-daily GHRH(1-29) has separately been studied in older men with circulating GH and IGF-I concentrations as endpoints.
